Breaking a Large Brick of Velium with a Smithing Chisel sometimes bugs out.
You end up with a duplicate smithing chisel and a small brick of velium. You can test which Chisel is the Real One, and apparently the 'other' smithing chisel is a small brick of velium that you can still work with.
For example, I broke a large brick into a Chisel, a Small Brick, and another Chisel.
I then put the Small Brick with Temper in the Forge and put in another chisel and hit Combine. Nothing happened. I took the OTHER chisel, put it in the forge with the Temper and One Small Brick, and made a Sheet of Velium.
The 'second chisel' was a small brick of velium, clearly.
This sort of client/server mixup used to happen frequently (well, more than 5% of the time) with smithing combines that returned multiple different objects. You'll find that if you relog to force the client to sync with the server the items will be shown correctly in your inventory. It's been a long time since I had it happen tho, might be more than two years, I thought they had laid the bug to rest. Hopefully it can be traced and fixed fairly quickly.

Broken down atleast 100 blocks all the way down to small pieces and havent had this issue happen once.

Good bug report.
However the real issue is the break down combines yielding 2 smaller portions of Velium. The classic combines only yielded 1. So instead of getting 4 small bricks out of 1 block of Velium, you would get 1 small brick.
This was changed in like GoD or later, here is a 2003 capture listing the break downs as yielding just 1 smaller brick:

The correct combines should be:
Block of Velium + Smithing Chisel = 1 Large Brick of Velium
Large Brick of Velium + Smithing Chisel = 1 Small Brick of Velium
The Small Brick -> Piece break down however classicly was 2 pieces.

I Get the client out of sync issue alot when I am making chain jointing for dwarf cultural. You have a smithing hammer, file, and jointing returning, and sometimes the file and jointing will be mixed up. This seems to happen to me about 1 out of 20 times still. They are always returned in a certain order, so I just remember what item it should be and just do the combines using the "wrong item". The server seems to know what items are what.
