I had two problems when turning in my pads / belts.

1. The quest npc took a pad without saying anything at all, he should have given me the "i reward for two kills, give me another!" speech but instead he said nothing! and when I gave him the 2nd one he gave me the "i reward for two kills" speech.

2. The quest npc will take ANY item you give him, even if it isn't related to the quest...

Pretty sure most NPCs take anything u give them. Here is another bug, I equipped the armor he gave me, after turning in 4 belts, and it logged me out lol.

Yes this bug has been brought up before. If you try and auto inventory a item when there are others on your cursor or you have no room or something like that and you crash. After you get the items from him just put them in your bags or on person manually and not try to auto inventory it.
