Remember the episode when Wesley was caught lying about performing the kolvoord starburst? Why was Tom Paris using a different name? They used the same character, but he had a different name on TNG...wtf?

These are all over the place. Apparently Leah Brahms retired from Utopia Planitia and changed her name, because in DS9 she showed up as Captain Benteen commanding the Lakota.
Also Mike Ehrmantraut survived 21st century New Mexico (along with Q) and went on to be a warlord on that one planet.

I think the Paris angle was perfect because he was already a troublemaker in starfleet academy

You bring up a great point. I always wondered what happened to him (Nicholas Locarno/Tom Paris) after the inquiry. Wesley had to repeat his year at the Academy and live with his shame, but that seemed like the extent of his punishment. Of course, Wes probably got a better deal than the rest of them since he was the snitch.
Perhaps we are to believe Locarno changed his name? I didn't watch all of Voyager. Does anyone know if they ever connected these dots or is this just writer laziness/lack of qualified actors who wanted to be part of another star trek?

From what I read it came down to having to pay royalties to the writers etc of that TNG episode.

No idea but fun fact, in Voyager they used a still shot from the TNG episode when Paris's father was shown looking at a picture of him when he was younger.

Locarno was expelled from the academy. At that point, nothing else matters.
And well... much more than a name change would be needed to make Paris be Locarno. We know Paris' dad was Admiral Owen Paris. Paris was never really a troublemaker, just arrogant. That arrogance probably led to his mistake that got three people killed and ended his career. And then he fell in with the Maquis, but they weren't all bad.
Sito Jaxa didn't get much punishment either. We see her later; no one wanted to fly with her so an instructor had to be her partner for a flight test. She then served on the Enterprise and was given a covert assignment against Cardassians by Worf. She's believed KIA.
